  • Synergy InnovationsCELERFORM: We developed a dynamic Form Generation platform to build a contract writing application for AF/AQCI to create PDS-compliant contracts. It achieved ATO on AF Platform One.




Celerform is a codeless platform for generating and publishing forms. No need for a developer—Celerform provides a web-based Designer for designing multi-page forms using an intuitive drag and drop interface. As you design a form, a preview window shows you, in real-time, the look and feel that the users will see in runtime. As a designer, Celerform lets you use your creative acumen to best present a data element. Want a dropdown instead of check boxes? Want to use a sliding scale to capture temperature rather than a typing in a number? No problem! Celerform gives you a library of UI controls to choose from, to render the same data. You can also define complex syntactic and semantic rules using the built-in rules engine to create the next generation of web forms. Once ready, you can publish the forms you designed to Celerform Runtime. Just share the link with your users to collect data in real-time!

Additionally, Celerform provides advanced enterprise-level features, such as version management and access control. Intuitive data management capability of Celerform Designer lets you ingest XML Schema Definitions (XSDs) and PDF, parse the data elements, and present them in a hierarchical tree. You can easily drag and drop those imported data elements into a form and define how to render them. These elements retain their data structure, which allows you to freely design forms while still maintaining any data lineage used in the imported schema. As a designer, you can also create your own data tree, or use a hybrid model. Data entered in the forms in runtime are stored as XML or JSON, complying with the initial data structure used to build the form.

Our mobile Population Health Management solution allows Care Coordinators to efficiently track and manage patient populations. CareDial helps identify high-risk patients to reduce overall frequency and duration of hospital stays, improves communication among Care Coordinators and provides reporting on the effectiveness of patient outreach programs.

Health IT

Synergy uses technology to improve patient engagement, improve quality of care delivery, and reduce costs of operation by offering expertise in developing solutions for Health IT interoperability, patient, and physician facing mobile apps, as well as insightful analytics to enable better healthcare delivery.
